Keep Your Medicaid Coverage

Important reminder: Keep your Medicaid coverage

Do you or a family member have Medicaid? If so, you need to take steps to keep coverage. Without coverage, you could end up paying much more for medical care and medicine.

Yearly eligibility reviews that were put on hold during COVID have now started. Members must complete an annual review each year to stay in the Healthy Connections Medicaid program.

Learn the easy steps you need to take to complete the Medicaid renewal process.

Act today to keep your Medicaid coverage:

  1. Make sure your contact information is up to date. Visit apply.scdhhs.gov or call 888-549-0820 to change your address, email or phone number. You don’t want to miss important updates about your coverage.
  2. Check your review status. Visit apply.scdhhs.gov or call 888-549-0820 to learn about your annual review date, when you should get your annual review form, whether your form has been received and more.
  3. Watch for your renewal notice. When it’s time to renew your coverage, you will receive notification by mail and text message (if your number is on file). The text will come from 803-879-4184. This is why it is important to make sure Healthy Connections Medicaid has your correct address and phone number. If you get a renewal form, complete it and return it within 30 days to continue your Medicaid coverage if you are still eligible.

What is needed to complete the eligibility review?

Below is the information you will need to complete the renewal form:
